Care Manager

Full Time

Attractive remuneration package offered, £35,000 subject to qualifications and experience including generous holiday allowance and pension contribution.

Based in St Davids, Pembrokeshire, West Wales

Closing date for applications:   24 July 2020

We are seeking an outstanding individual who shares our values and ambitions, who will embrace an opportunity to develop a new concept in local care for those diagnosed with life limiting or palliative conditions, and to become a recognised centre of excellence in living well with cancer and other life limiting conditions.

This is an exciting opportunity to participate in the next phase of the development of Shalom House, as it moves from a palliative care day centre to a centre for living well with life limiting conditions.  Services will be expanded not only to include our traditional palliative services but also to support patients at home by working closely with our partners in the community.  Your expertise in the organisation of domiciliary services together with your drive and enthusiasm to develop and provide a range of individual patient-led services will be invaluable to ensure the future of this much-loved charity in Pembrokeshire.

Shalom House is a charity based in St Davids, which offers professional support to people in Pembrokeshire with life-limiting illness and to the family and friends who care for them.  Shalom House is non-denominational and takes its name from the literal meaning “peace.”  We help people to live well with cancer and other serious conditions. We aim to enable all patients to manage their diagnosis to do the things that matter to them and to make their lives more comfortable than they otherwise would have been.  Our holistic care recognises the grief and loss of bereavement as we support patients and carers through their journey.  Our house in St Davids also provides a base for a range of complementary health services to support the local community.

Our clinical services are provided by Registered Nurses and Occupational Therapist, supported by qualified Health Care Assistants, registered with Social Care Wales.  The new organisation will be led by the Care Manager, responsible for all clinical aspects of the charity, who will report to the Board of Trustees, together with the new post of Commercial Manager, who will manage all other business aspects of the charity including income generation.
